Required Forms for ALL Students

The below forms need to be completed prior to a student starting school at Denfeld.  In order to complete the forms, a parent/guardian must "create a ParentSquare Account."

• Locker Agreement (Available Aug. 12)
• Household Verification Form (Available Aug. 13)
• Annual Health History Form (Available Aug. 13)
• Blended Learning Agreement (Available now)
• Dance Contract (Available Aug. 15)
