Athletic Registration Information

Minnesota State High School League (MSHSL) Eligibility and ISD709 Policies/Regulations Online Form

Athletic registration is to be completed online and is required for each activity your student participates in. 

Your child will not be allowed to participate without this form completed.

Online Registration is REQUIRED by all participants.

Please have the following information ready before starting the registration process:

• - ISD709 Student ID Number (Lunch Number) *This is NOT needed for homeschooled students (Just be sure to select Homeschool in the registration form)
• - Contact Information (E-mail Addresses, Phone Numbers)
• - Emergency Contact Information
• - Family Medical History
• - Health/Dental Insurance Information
•  MSHSL Sports Physical Form completed if you do NOT have one on file in the Athletic office at Denfeld. This must be completed be your physician every 3 years unless otherwise specified by your physician. Must be completed after your 6th grade school year.

Transfer and Exchange Students

Transfer or Exchange Students - Contact the AD by either stopping in to the office or via email at Tom.Pearson@isd709.org

Transfers and exchange students will not be able to practice or participate until they are cleared for play by the Duluth Denfeld Activities Office and the MSHSL.

FAQ's for Activity Registration

• You will be required to create a Family/Household account and verify using the email assigned to the account. 
• When you are asked if you would like to purchase supplemental insurance for $21... If you select yes, you will be required to pay that online payment, I believe that you will need to pay it prior to submitting your registration. Please note that this insurance is NOT associated in any way with ISD 709 or Denfeld High School. 
• Any medical information/medications are visible for coaches, incase they need that information during the season.
• If you qualify for Free or reduced activity fees you will be asked to check a box stated so.
