Athletic Registration Information

Minnesota State High School League (MSHSL) Eligibility and ISD709 Policies/Regulations Online Form

Athletic registration is to be completed online. 

Your child will not be allowed to participate after Friday, August 16th without this form completed.

Online Registration is REQUIRED by all participants.

Please have the following information ready before starting the registration process:

  • - ISD709 Student ID Number (Lunch Number) *This is NOT needed for homeschooled students
  • - Contact Information (E-mail Addresses, Phone Numbers)
  • - Emergency Contact Information
  • - Family Medical History
  • - Health/Dental Insurance Information
  • 2024-25 MSHSL Sports Physical Form completed if you do NOT have one on file in the Athletic office at Denfeld.

2024-2025 MSHSL Sport Qualifying Physical

DUE BY WEDNESDAY, AUGUST 14TH

You CANNOT practice or compete without a current physical.

Every student athlete is required to have a valid sports physical on file with the Denfeld Athletic Office prior to any participation in any sport. Make sure your students physical is up to date. They are typically valid for 3 years unless your students physician states differently. You must print this out and bring it to your physicians office to be filled out. There are copies in the Athletic Office if you do not have access to a printer.  

NO PHYSICAL = NO PLAY!

Your students' physical status (expiration date) can be found in your Family Account on the Activity Registration site

You can upload your physical during the online registration or bring it into the Activities Department Offices

SWAY Concussion Testing

ISD709 and Essentia Health Athletic Trainers Recommend that every athlete complete the SWAY Concussion preliminary exam prior to the start of their sport.  Please view this flier to get your code. Each grade has a distinct code.

2024-2025 Application for Benefits for Free or reduced Athletic/Activity Fees

Reduced parcipitation fees for those that qualify

If you are requesting reduced parcipitation fees for any athletics and /or activities you MUST apply for benefits using this form and return it to Child Nutrition prior to the Athletic season. A new form must be completed each year that you are requesting assistance. 

You may fill out the form on your Infinite Campus Parent Portal or by printing the attached PDF and returning it to the Child Nutrition Office

Athletic / Activity Fees

No payments are required until after team selections have been made, at that time payments can be made in student's Athletic/Activity account under payments OR cash / check can be turned into the athletic offices (please make sure to include you students first & last name along with activity they are participating for). If you choose to pay via CC, this must be done through the Athletic / Activity registration site and there is a convenience fee.

Include your student's first and last name and activity you are paying for is with or on your check

  • All participation fees must be paid prior to the first scheduled event.
  • The participation fee is reduced to students who qualify for reduced lunch to $25.00 for each activity.
  • The participation fee is waived for students who qualify for free lunch.


Family Limits $1,000

Participation Fee Refunds:  Students who choose to participate in athletics must realize there are no guarantees regarding playing time or injuries. No participation fees will be refunded after the day prior to the first scheduled event.

Travel Release Form

If your student is not riding on the district provided transportation either to or from an event, this form needs to be filled out and brought to the athletic offices in Denfeld prior to the event.
